The following will be available through a co-pay or 
co-insurance at the time of service:
 
  • Up to three (3) visits per year to a Primary Care Physician
  • Up to three (3) visits per year to a Specialist
  • Up to three (3) visits per year to an Urgent Care Facility
  • In office Diagnostic tests
  • X-Ray, CT Scan, and MRI outpatient testing 
  • Prescription drug benefits
